About Us

 

Formed in 2019 we are a PCN located in the South of Croydon across three areas, Selsdon, Purley and Coulsdon.

The origins of the formation of this PCN group start in 2019. As part of the NHS Long Term Plan, a key element of the new GP Contract was the development of Primary Care Networks (PCNs) which bring GP practices together with other local services; such as community, mental health, social care and the voluntary sector to look after local populations. PCNs help to join up services at a local level, focusing on the specific needs of these local populations with patients still being able to access routine GP appointments as they do now.

Our surgery teams are working closely with each other, enjoying the ability to share expertise and resources and to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the need of our patient population. Because we are part of a PCN we are able to take advantage of additional staff roles that are now available to support all of our patients. These additional roles help us to provide the right care at the right time, from the right professional. New roles include Advanced Nurse Practitioners, Clinical Pharmacists, Physiotherapists and Social Prescribing Link Workers.

Mitchley Avenue has since merged with Selsdon Park Medical Practice. We serve circa 55,000 patients in our community.

We are a pioneering group and are proud to have been the first PCN with Croydon to role out the COVID-19 vaccination at the height of the pandemic. Our team is based placed for collaborative working and digital innovation to provide the best care for our patients.
